If there were no upfront payments then no need to file a claim/reimbursement. However, if reimbursement is necessary, we request that you provide us with the necessary documents for processing, including a Medical Certificate, Statement of Account (SOA), Valid ID, Etiqa Outpatient Form, and Receipts.
